New Patients:   
PLEASE PLACE AN ONLINE REQUEST  HERE - you will be asked to complete a screening form including a few medical history questions which will remain completely confidential. Forms may be directly uploaded and will become part of your future visit.  

Existing Patients: It is most efficient to message with your concerns via the patient portal. Please be aware that calls and emails will typically be responded to within one business day. Refill requests are typically handled during appointments. A 48 business hour window may be necessary to handle refill requests between appointments. 
 
Emergencies/After hours:  If you have an urgent medical or psychiatric concern that cannot wait outside of normal business hours, M-F 8a to 5p, please go to the closest Emergency Room. If you have a concern that you or someone you love is not safe, you may also consider calling the Colorado Crisis Services line at 1-844-493-8255, text TALK to 38255, OR calling 911.
